METHOXYFLURANE IN MAJOR ORAL SURGERY BY V. GOLDMAN AND P. B. HARDWICK SUMMARY A series of cases undergoing major oral surgery such as mandibular osteotomy or ostectomy is described, using methoxyflurane as the main anaesthetic agent with nitrous

چکیده
The use of methoxyflurane (Penthrane) as an anaesthetic agent has not been popular in this country for a number of reasons. Foremost among these have been the prolonged induction time (Thomason, Wright and Holaday, 1962) and the delayed rate of recovery (Campbell, Hvolboll and Brechner, 1962), both due to the low vapour pressure and blood solubility of this agent. Because of its high lipoid solubility, however, methoxyflurane is a potent and good analgesic drug and has been used in obstetrics for this sole reason (Major, Rosen and Mushin, 1966). In consequence of the lengthy induction time, its main use is limited to long surgical procedures, but its delayed time of recovery and sustained analgesic properties make it a suitable drug for use in cases in which these are desirable effects.

This is the second of 2 articles evaluating cardiac events in patients undergoing noncardiac surgery. Unrecognized myocardial infarctions (MIs) are common, and up to 50% of perioperative MIs may go unrecognized if physicians rely only on clinical signs or symptoms. In this article, we summarize the evidence regarding monitoring strategies for perioperative MI in patients undergoing noncardiac s...
متن کاملIdentifying risk factors for postoperative cardiovascular and respiratory complications after major oral cancer surgery.
BACKGROUND Surgical resection of oral cancer can be associated with significant postoperative cardiovascular and respiratory complications that require more sensitive predictors. METHODS All patients with oral squamous cell carcinoma treated from July 2005 to April 2008 were retrospectively reviewed. The Goldman Revised Cardiac Risk Index (GRCRI) was used to predict cardiovascular complicatio...

CONTEXT Right heart catheterization (RHC) is commonly performed before high-risk noncardiac surgery, but the benefit of this strategy remains unproven. OBJECTIVE To evaluate the relationship between use of perioperative RHC and postoperative cardiac complication rates in patients undergoing major noncardiac surgery. DESIGN Prospective, observational cohort study. SETTING Tertiary care tea...
متن کاملDynamic contour tonometry and goldman applanation tonometry in eyes with keratoconus.
PURPOSE The dynamic contour tonometer is a nonapplanation contact tonometer designed to be largely independent of the structural properties of the cornea. Theoretically, it may measure intraocular pressure most accurately in abnormally thinner corneas. This study compares intraocular pressure measurements by dynamic contour tonometry with Goldman applanation tonometry in eyes with normal cornea...
